Truscum are people who take the view that transgenderness requires both physical dysphoria and a wish for medical intervention. They also often promote binary essentialism, i.e. the idea that nonbinary people either are not really trans or don't actually exist.
This exclusion of some types of trans people from the designation "trans" is very similar to how TERF cis women exclude trans women from the designation "women".
